+Please attach a page to your assignment or test describing your concerns and hand it to the instructor after class. The instructor will (together with the TAs) reevaluate your work and make any changes. The whole assignment or test may be reevaluated. Please provide the instructor with your written comments within **one week** of receiving your assignment or test back.

+If you are not able to write the test due to a serious medical emergency, then you must ask your physician to fill out the standard medical form (available from the departmental office). In this case, the final exam will count as 60% of your final grade (45% + 15%).
