===== Missed Midterm Test and Quiz ===== 

     * Contact the instructor immediately and submit acceptable documents  (e.g., [[https://www.eecs.yorku.ca/course_archive/2012-13/W/2001/PhysSt.pdf|"Attending Physician's Statement"]] ).

     * The Midterm grade or quiz will be either zero or the Final Exam grade as decided by the instructor based on submitted documents.

     * Contact the instructor immediately and submit acceptable documents  (e.g., [[https://www.eecs.yorku.ca/course_archive/2012-13/W/2001/PhysSt.pdf|"Attending Physician's Statement"]] ).
     * You may or may not be granted a deferred exam, as decided by the instructor based on submitted documents.
     * If not granted, you will get a zero for the exam. (You have the right to appeal.)
